Home Delivered Meals, located in Cranberry Township, PA, is dedicated to providing essential nutrition through its Home Delivered Meals and Medically Tailored Meals programs. We serve eligible clients covered by Medicare Advantage plans, Medicaid Long-Term Services and Supports (LTSS), and the Older Americans Act, ensuring that those in need receive meals thoughtfully designed by a Registered Dietitian. Our meals cater to specific nutritional requirements for various health conditions, including Chronic Kidney Disease and Congestive Heart Failure, while being mindful of dietary restrictions, as they are free from dairy, peanuts, gluten, and pork. We also offer a self-pay option at an affordable rate of $7.75 per meal for individuals not qualifying for assistance. To determine eligibility for meal delivery services, clients are encouraged to consult their Case Manager or local Area Agency on Aging. Additionally, our services include Medical Nutrition Therapy and Nutrition Counseling, enhancing the well-being of our community through comprehensive care.
