The Blood Pressure Self-Monitoring program at the YMCA in Camp Hill, PA, is designed to empower individuals with the knowledge and tools necessary for effectively managing their blood pressure. Through a personalized approach, participants are guided to establish a consistent habit of monitoring their blood pressure, while receiving evidence-based support for chronic disease prevention. Participants will take and record their blood pressure at least twice each month, engage in bi-monthly consultations with a dedicated Healthy Heart Ambassador, and attend monthly nutrition education seminars.
