Our Home and Community Based ADvantage Waiver program offers a comprehensive range of essential services designed to keep individuals out of nursing facilities. These services include personalized case management, nursing care, home-delivered meals, assistance with specialized medical equipment, prescription medication management, home modifications, and foster care options such as assisted living and adult day health services. Additionally, we provide compassionate hospice care to support our clients and their families during challenging times.
