ADvantage proudly serves the Perry, OK community by offering the Home and Community Based ADvantage Waiver program, designed to provide essential alternatives for eligible adults who wish to maintain their independence and stay connected with family and friends, instead of being placed in a nursing facility. Our comprehensive services include case management, personal care, skilled nursing, home-delivered meals, specialized medical equipment, prescription medications, home modifications, assisted living, adult day health, and hospice care. We are dedicated to empowering older adults and individuals with disabilities to live comfortably in their own homes while receiving the support they need. Applications are available online for convenience, and assistance is provided for those who require help completing the application process.
