ADvantage, located in Muldrow, OK, is dedicated to empowering older adults and individuals with disabilities to live independently and remain connected to their loved ones. Our Home and Community Based ADvantage Waiver program offers essential services such as case management, personal care, skilled nursing, and home-delivered meals, along with specialized medical equipment and supplies. We also provide support with prescription medications, home modifications, assisted living, adult day health, and hospice care. We understand the importance of staying in familiar surroundings, and we are here to help you navigate the pathway to independence. Applications for our services can be conveniently completed online, and those needing assistance can contact the Medicaid Services Unit to set up a phone application appointment.
