ADvantage, located in Mcloud, OK, is committed to empowering eligible adults, particularly older adults and individuals with disabilities, to live independently while remaining close to their loved ones. Our Home and Community Based ADvantage Waiver program offers essential services to support this mission, including personalized case management, skilled nursing care, home-delivered meals, and access to specialized medical equipment and supplies. We also provide assistance with prescription medications, home modifications, adult day health, hospice care, and opportunities for assisted living. By focusing on individual needs, we strive to enhance quality of life and promote independence for our clients. Applications for our services can be easily completed online, and those needing additional assistance are encouraged to reach out to our Medicaid Services Unit to schedule a telephone application appointment.
