Through the Home and Community Based ADvantage Waiver program, we offer a comprehensive range of services designed to support their specific needs, including case management, personal care, skilled nursing, home-delivered meals, specialized medical equipment, and modified living environments. Our services also encompass prescription medications, assisted living, adult day health, and hospice care, all aimed at fostering a comfortable and dignified life at home.
