ADvantage, based in Choctaw, OK, is dedicated to empowering older adults and individuals with disabilities to maintain their independence and stay connected with their loved ones. Through our Home and Community Based ADvantage Waiver program, we offer a comprehensive range of services, including case management, personal care, skilled nursing, home-delivered meals, specialized medical equipment, and prescription medications. We also provide home modifications, assisted living, adult day health, and hospice care to enhance the quality of life for our participants. Our mission is to facilitate access to essential support while ensuring a compassionate and secure environment. Applications for our services can be completed online, and for those who require assistance, we offer the option to schedule a phone appointment with our Medicaid Services Unit.
