ADvantage is dedicated to empowering older adults and individuals with disabilities in Catoosa, OK, to maintain their independence and stay connected with loved ones by offering the Home and Community Based ADvantage Waiver program. This supportive initiative provides a range of essential services, including case management, personal care, skilled nursing, home-delivered meals, specialized medical equipment and supplies, prescription medications, home modifications, assisted living, adult day health, and hospice care. Our mission is to ensure that eligible individuals have access to comprehensive resources that enhance their quality of life, all while remaining in the comfort of their own homes. Applications are available online for convenience, and for those who prefer, assistance in completing the application via telephone can be arranged through the Medicaid Services Unit.
