Our Home and Community Based ADvantage Waiver program offers vital services including case management, personal care, skilled nursing, and home-delivered meals to ensure that older adults and individuals with disabilities can thrive in their communities. We also provide specialized medical equipment, prescription medications, home modifications, assisted living options, adult day health services, and hospice care to meet the diverse needs of our clients.
