Through our Home and Community Based ADvantage Waiver program, we offer a range of vital services, including personalized case management, skilled nursing care, home-delivered meals, assistance with obtaining specialized medical equipment, and support for prescription medications. Our services extend to home modifications, assisted living options, adult day health, and hospice care, ensuring that our clients receive comprehensive assistance tailored to their needs. We invite eligible individuals to apply online, and those who require help with the application process can schedule a telephone appointment with our Medicaid Services Unit.
