ADvantage is dedicated to empowering older adults and individuals with disabilities in Afton, OK, by offering a comprehensive Home and Community Based Waiver program that provides essential services to support independence and connection with loved ones. Our mission is to ensure that eligible adults have access to a variety of critical resources, including personalized case management, skilled nursing, and home-delivered meals. In addition, we facilitate access to necessary medical equipment, prescription medications, home modifications, assisted living, adult day health services, and hospice care. We believe in helping individuals remain in their communities and close to their families while receiving the care they need. Applications for our services can be conveniently completed online, or for those needing assistance, a telephone application process is available through our Medicaid Services Unit.
