PASSPORT Program

The PASSPORT Program in Toledo, OH, is dedicated to empowering frail and low-income older adults to maintain their independence in the comfort of their own homes. We offer a comprehensive suite of in-home care services tailored for individuals aged 60 and above, allowing them to choose their own service providers, which can include friends, neighbors, and sometimes family members. Our services encompass meal delivery, personal care assistance, housekeeping, transportation, emergency response systems, and access to home medical equipment and supplies. Additionally, we provide adult day services and minor home modifications to enhance safety and comfort. Available to those eligible for Medicaid's Assisted Living Waiver, our mission is to ensure that older adults in Toledo receive the compassionate support they need to thrive in their own living environments.
