Ohio Home Care Program

The Ohio Home Care Program, based in Oxford, OH, is dedicated to providing essential support and care for individuals with chronic conditions and disabilities across all 88 counties in Ohio. Our program focuses on empowering people to receive personalized services in the comfort of their homes and communities, rather than in institutional settings. We offer a range of services including skilled nursing care, personal care, homemaking help, home-delivered meals, and personal emergency response systems. Our dedicated team also facilitates home modifications to improve accessibility, provides maintenance for minor repairs, and offers specialized medical equipment as well as vehicle modifications. We understand the importance of respite and community engagement, which is why we provide out-of-home respite care, adult day care, and supplemental transportation. Through the Ohio Home Care Waiver Program, we aim to enhance the quality of life for our clients while prioritizing their independence and dignity.
