Ohio Home Care Program

At CareStar, we are dedicated to empowering individuals in Lebanon, OH and across all 88 counties of Ohio through our Ohio Home Care Program. Our mission is to provide essential assessment and case management services that help individuals with chronic conditions and disabilities access the care they need in the comfort of their own homes. We offer a comprehensive range of services including skilled nursing care, personal care, homemaking assistance, home-delivered meals, and personal emergency response systems. Additionally, we facilitate accessible home modifications, minor home repairs, specialized medical equipment, and vehicle adaptations to enhance each individual's quality of life. Through our program, we also provide respite care, adult day care options, and supplemental transportation, ensuring families receive the support they need. Our goal is to enable Ohio residents to live independently and maintain their dignity and safety at home, connecting them to the right resources and services they deserve.
