Ohio Home Care Program

The Ohio Home Care Program, based in Batavia, Ohio, is dedicated to supporting individuals with chronic conditions and disabilities by helping them receive essential care in the comfort of their own homes. As part of the Ohio Department of Medicaid’s waiver services, we offer a comprehensive range of services, including skilled nursing care, personal care, and homemaking assistance, along with home-delivered meals and personal emergency response systems. Our program also provides critical home modifications, maintenance services for minor repairs, specialized medical equipment, and out-of-home respite care. Additionally, we facilitate adult day care and supplemental transportation, ensuring that our clients have access to the support they need to thrive in their communities. Through personalized assessment and case management services across all 88 counties in Ohio, we are committed to enhancing the quality of life for those we serve by enabling them to remain in their homes and live independently.
