Ohio Home Care Program

The Ohio Home Care Program, based in Ashland, OH, is dedicated to enhancing the quality of life for individuals with chronic conditions and disabilities by providing essential in-home care services. Through skilled nursing, personal care, and homemaking assistance, we ensure that our clients receive the support they need to thrive in their own homes. Our program also offers vital services such as home-delivered meals, personal emergency response systems, accessible home modifications, and minor home repair assistance. Additionally, we facilitate out-of-home respite care, adult day care, supplemental transportation, and provide access to specialized medical equipment and vehicle modifications. As part of the Ohio Department of Medicaid’s waiver services, our mission is to empower individuals throughout all 88 counties of Ohio to remain in their communities, enjoying the comfort and freedom of home-based care.
