We offer free, home-delivered, medically-tailored meals for individuals living with chronic illnesses and physical limitations, ensuring they receive the nutrition they need to thrive. Our program delivers lunch and dinner five days a week and includes ongoing nutritional counseling to cater to each client's unique medical requirements. Meals are delivered directly to homes, one or two days a week, based on the client’s zip code, with delivery details confirmed during the enrollment process. We invite those in need to apply for this essential service by downloading the required forms from our website, having them signed by a medical care provider, and returning them via fax or email, or by contacting us directly to assist with the application process.
