Diabetes Education Program

Located in Smithtown, NY, the Diabetes Education Program is dedicated to providing essential support and comprehensive education for individuals managing diabetes. Our program connects you with registered dietitians who offer valuable healthy eating advice, meal planning assistance, and strategies for understanding food labels and weight management. Additionally, our diabetes nurse educators are here to enhance your understanding of diabetes management, including the use of blood glucose meters, insulin, and medication. We also offer engaging Diabetes Self-Management Classes, a four-part series designed to empower participants with effective strategies for living well with diabetes, while providing a supportive environment to share experiences with others. Our classes are open to the community and are held in the Spring and Fall, making it easy for everyone to access vital resources and support.
