Diabetes Education Program

Located in Patchogue, NY, the Diabetes Education Program is dedicated to empowering individuals with the knowledge and resources needed to manage diabetes effectively. Our program provides personalized support, connecting participants with registered dietitians who offer guidance on healthy eating, meal planning, and reading food labels. Additionally, diabetes nurse educators are here to assist with understanding diabetes, using blood glucose meters, and managing medications and insulin. We also offer comprehensive Diabetes Self-Management Classes, held in the Spring and Fall, where you can learn practical strategies for diabetes management while connecting with others who share similar experiences.
