Located in East Meadow, NY, Cancer Transitions offers a compassionate, evidence-based program designed specifically for cancer survivors who have completed their treatment. Aimed primarily at individuals within 24 months post-treatment, this initiative recognizes the ongoing needs of all survivors, regardless of how long it has been since treatment completion. Our comprehensive program focuses on essential aspects of survivorship, including nutritional support to promote health, customized exercise plans to enhance physical well-being, education on emotional health, and guidance on medical management.
