Jersey Assistance for Community Caregiving (JACC)

Jersey Assistance for Community Caregiving (JACC), located in Union, NJ, is a compassionate State-funded program dedicated to supporting individuals at risk of nursing facility placement. By offering a variety of essential in-home services, JACC empowers participants to maintain their independence while remaining in their own homes. Our comprehensive array of services includes care management, home health aide support, respite care, home modifications, and access to special medical equipment. We also provide chore services, personal emergency response systems, and home-delivered meal services, along with caregiver training and social adult day care.
