Jersey Assistance to Community Caregivers Program (JACC)

Located in Lakewood, NJ, the Jersey Assistance to Community Caregivers Program (JACC) is dedicated to enhancing the quality of life for seniors at risk of nursing facility placement by providing essential in-home services. Our comprehensive program is designed to support the vital work of family, friends, and neighbors who serve as caregivers. We offer a range of services, including respite care, home health aide assistance, environmental accessibility adaptations, personal emergency response systems (PERS), home-delivered meals, social adult day care, adult day health services, special medical equipment and supplies, transportation, and chore services. At JACC, we prioritize the dignity and independence of seniors, ensuring they receive the care they need within the comfort of their own homes.
