Nutrition for Longevity (N4L) is dedicated to enhancing health and well-being in Lakewood, NJ, through our Home Delivered Meals and Medically Tailored Meals programs. We serve eligible clients covered under Medicare Advantage plans, Medicaid Long-Term Services and Supports (LTSS), the Older Americans Act, and offer a self-pay option for individual clients seeking nutritious meals. Our meals, meticulously planned by a Registered Dietitian, cater to various dietary needs and medical conditions such as Chronic Kidney Disease and Congestive Heart Failure, ensuring they are free from dairy, peanuts, gluten, and pork. Eligibility for meal delivery may vary, so we encourage those interested to consult their Case Manager, Medicaid provider, or local Area Agency on Aging for assistance. For Medicare Advantage beneficiaries, check your plan materials, as some include meal delivery as part of their support services. Additionally, our self-pay program allows individuals to select meals for just $7.75 each, making it accessible for those not covered by insurance. At N4L, we also offer Medical Nutrition Therapy and Nutrition Counseling to promote optimal health.
