Jersey Assistance for Community Caregiving (JACC)

Jersey Assistance for Community Caregiving (JACC) is dedicated to empowering individuals in Elizabethport, NJ, who are at risk of nursing facility placement by offering comprehensive in-home services tailored to their unique needs. Our program prioritizes keeping clients in their community homes through essential support such as care management, home health aides, respite care, and home modifications. We also provide special medical equipment, chore services, a personal emergency response system, and home-delivered meal services. In addition, our training programs for both caregivers and recipients, along with social adult day care options, foster a strong community connection. JACC operates on a sliding scale for co-pays based on income, ensuring that financial barriers do not prevent access to critical care. Our mission is to enhance quality of life by allowing individuals to live independently and safely at home while receiving the care they deserve.
