Jersey Assistance to Community Caregivers Program (JACC)

Based in Clifton, NJ, the Jersey Assistance to Community Caregivers Program (JACC) is dedicated to supporting seniors who are at risk of needing nursing facility placement. Our mission is to enrich the lives of older adults and their caregivers by providing essential in-home services that complement the love and assistance offered by family, friends, and neighbors. JACC offers a diverse range of services, including respite care, home health aides, environmental accessibility adaptations, personal emergency response systems, home-delivered meals, social adult day care, adult day health services, specialized medical equipment, transportation, and chore services. Our priority is to empower seniors to maintain their independence and access the support they need to thrive at home.
