Based in Elizabeth City, NC, Cancer Transitions is dedicated to supporting cancer survivors who have recently completed treatment. Our evidence-based program is designed for those who have finished their treatment within the past 24 months, although we encourage any survivor to participate, finding value at any point in their healing journey. We focus on holistic well-being by offering personalized nutritional guidance, customized exercise plans, and crucial emotional health education. Additionally, our program provides valuable resources for medical management, empowering survivors to take charge of their health and thrive during their survivorship.
