Home Delivered Meals

Located in Waterford, MI, Nutrition for Longevity (N4L) is dedicated to enhancing the well-being of our community through our Home Delivered Meals and Medically Tailored Meals program. We serve eligible individuals covered by Medicare Advantage, Medicaid Long-Term Services and Supports (LTSS), and the Older Americans Act, as well as self-pay clients seeking nutritious meal options. Our meals, crafted by Registered Dietitians, cater to specific dietary needs, supporting health conditions like Chronic Kidney Disease, End Stage Renal Disease, and Congestive Heart Failure. Each meal is designed to meet essential nutrient requirements while being free from dairy, peanuts, gluten, and pork. We invite those who may be eligible for meal delivery services to connect with their Case Manager or local Area Agency on Aging for assistance. For Medicare Advantage beneficiaries, it’s important to check your plan details for meal program eligibility. For those who don’t qualify for assistance, we offer a self-pay option at an affordable rate of $7.75 per meal, ensuring everyone can access convenient nutrition. Additionally, N4L provides Medical Nutrition Therapy and Nutrition Counseling to further support health goals. Together, we nurture health and longevity, one meal at a time.
