Our mission is to facilitate small, achievable health changes that lead to lasting improvements, providing essential services like health intake screenings, goal-setting, and ongoing support from compassionate health coaches. We strive to make a positive impact by connecting participants with community-based lifestyle programs, including the Diabetes Prevention Program, which we fully fund.
