Nurse-Family Partnership

Nurse-Family Partnership is dedicated to supporting first-time mothers in Pontiac, Southfield, Oak Park, Hazel Park, and Madison Heights who are less than 28 weeks pregnant. Our program connects you with a compassionate Public Health Nurse who will personally visit your home throughout your pregnancy and continue these vital visits until your child reaches two years of age. We focus on promoting a healthy pregnancy and a thriving baby, while helping you become a confident parent and establishing a strong support network. Our nurses also provide guidance on creating a safe home environment, accessing healthcare and childcare, pursuing education and job training opportunities, and achieving your family's goals for the future.
