Nutrition for Longevity (N4L) proudly serves the Monroe, MI community by offering Home Delivered Meals and Medically Tailored Meals designed to support the health and well-being of our clients. We cater to eligible individuals covered by Medicare Advantage plans, Medicaid Long-Term Services and Supports (LTSS), and the Older Americans Act, as well as those who wish to utilize our self-pay program. Our meals, meticulously planned by a Registered Dietitian, address a variety of dietary needs for conditions like Chronic Kidney Disease and Congestive Heart Failure, while being formulated without dairy, peanuts, gluten, or pork. We acknowledge the diverse needs of our clients by providing meal delivery options that vary by state and program, ensuring that beneficiaries can seamlessly access our services through their Case Managers or local Area Agencies on Aging. For individuals enrolled in select Medicare Advantage PPO plans, meal support is available as part of chronic care assistance. Additionally, our self-pay option allows anyone to enjoy the convenience of home-delivered meals at an affordable rate of $7.75 per meal, with the flexibility to choose each meal and delivery. At N4L, we are committed to enhancing lives through nutritious food and offer Medical Nutrition Therapy and Nutrition Counseling to further support our clients' health goals.
