Home Delivered Meals, a service of Nutrition for Longevity in Lapeer, MI, is dedicated to providing nutritious Home Delivered and Medically Tailored Meals to clients who qualify under Medicare Advantage, Medicaid Long-Term Services and Supports, or the Older Americans Act. Our meals are meticulously designed by a Registered Dietitian to support a wide range of health needs, including Chronic Kidney Disease and Congestive Heart Failure, ensuring clients receive the essential nutrients for their well-being. We pride ourselves on offering meals that are free from dairy, peanuts, gluten, and pork, making them suitable for various dietary restrictions. For beneficiaries of Medicaid and the Older Americans Act, meal delivery eligibility is determined by specific state and program requirements, which can be clarified through a Case Manager or local Area Agency on Aging. We also serve Medicare Advantage beneficiaries through select PPO plans that include meal provisions. For those who prefer a self-pay option, our service is available for just $7.75 per meal, providing the opportunity to choose every meal and delivery that fits your needs. Additionally, Nutrition for Longevity offers Medical Nutrition Therapy and Nutrition Counseling to enhance our clients' health journeys.
