Cancer Transitions is dedicated to supporting cancer survivors in Farmington, MI, who have recently completed their treatment. Our evidence-based program is designed for individuals up to 24 months post-treatment, though it can provide valuable resources for survivors at any stage of their journey. We focus on enhancing quality of life through personalized nutritional support, customized exercise plans, emotional health education, and comprehensive medical management guidance. Our mission is to empower cancer survivors with the knowledge and skills they need to thrive and navigate their survivorship with confidence and care.
