Located in Eastpointe, MI, Nutrition for Longevity (N4L) is dedicated to delivering nutritious, home-cooked meals to individuals in need, ensuring that dietary support is accessible for those covered by Medicare Advantage, Medicaid Long-Term Services and Supports (LTSS), or the Older Americans Act. Our Home Delivered Meals program features expertly planned meals by registered dietitians tailored to meet the specific nutritional needs of clients coping with various medical conditions such as Chronic Kidney Disease and Congestive Heart Failure. We take pride in offering meals that are free from dairy, peanuts, gluten, and pork, emphasizing health and well-being. For those eligible under Medicaid and Older Americans Act programs, meal delivery service is available, and we encourage individuals to check their benefits with a Case Manager or local Area Agency on Aging. Medicare Advantage beneficiaries may also find meal options included in their care plans, specifically within PPO plans. Additionally, we offer a self-pay program for those seeking the convenience of home-delivered meals at an affordable rate of $7.75 per meal, with the flexibility to choose every meal and delivery. Beyond meals, N4L provides Medical Nutrition Therapy and Nutrition Counseling, with coverage dependent on your health plan, further supporting the nutritional health of our community.
