In Commerce Township, MI, Nutrition for Longevity (N4L) is dedicated to providing vital Home Delivered Meals and Medically Tailored Meals to individuals eligible under Medicare Advantage plans, Medicaid Long-Term Services and Supports, and the Older Americans Act. Our program is designed to enhance health and well-being by offering meals carefully crafted by a Registered Dietitian, addressing a variety of dietary needs for conditions such as Chronic Kidney Disease and Congestive Heart Failure. We pride ourselves on offering meals that are free from dairy, peanuts, gluten, and pork, ensuring a safe dining experience for all our clients. N4L is committed to serving Medicaid and Older Americans Act beneficiaries, with eligibility varying by state and program—clients can verify meal delivery options with their Case Managers or local Area Agencies on Aging. For Medicare Advantage insurance holders, we provide meals as part of post-discharge support for certain plans. Additionally, we offer a self-pay option at $7.75 per meal for those without insurance or who may not qualify for assistance, allowing clients to select their meals and delivery times for added convenience. Beyond meal delivery, Nutrition for Longevity also provides Medical Nutrition Therapy and Nutrition Counseling, helping clients manage their health and dietary needs effectively.
