Home Delivered Meals

Nutrition for Longevity (N4L) in Bay City, MI, is dedicated to enhancing health and well-being through our Home Delivered Meals program, specializing in Medically Tailored Meals for individuals eligible under Medicare Advantage plans, Medicaid Long-Term Services and Supports, or the Older Americans Act. Our meals, designed by a Registered Dietitian, cater to diverse nutritional needs, including support for conditions such as Chronic Kidney Disease and Congestive Heart Failure, and are free from dairy, peanuts, gluten, and pork. For those who qualify under Medicaid or the Older Americans Act, our meals are provided at no cost, and assistance is available through your Case Manager or local Area Agency on Aging. We also support Medicare Advantage beneficiaries with certain plans that include meal delivery as part of their care services. For those not covered by these programs, we welcome self-pay individuals to enjoy the convenience of our home-delivered meals for just $7.75 each, with a selection that allows you to choose every meal and delivery. Additionally, N4L provides Medical Nutrition Therapy and Nutrition Counseling, with coverage details available through your insurance plan.
