Located in Alpena, MI, Nutrition for Longevity (N4L) is dedicated to providing Home Delivered Meals and Medically Tailored Meals to individuals eligible under Medicare Advantage plans, Medicaid Long-Term Services and Supports (LTSS) programs, and the Older Americans Act. Our program stands out by offering meals designed by a Registered Dietitian to address both general health needs and specific medical conditions such as Chronic Kidney Disease, End Stage Renal Disease, and Congestive Heart Failure. We ensure our meals cater to food sensitivities, being free from dairy, peanuts, gluten, and pork. For those receiving Medicaid or Older Americans Act benefits, our services can be tailored based on state-specific eligibility, and we encourage connecting with your Case Manager for detailed support. Beneficiaries of some Medicare Advantage plans may also be eligible for our meals as part of their post-discharge or chronic care support. For individuals or caregivers seeking nutritious options without health plan coverage, our self-pay program offers personalized meal choices at an affordable rate of $7.75 per meal. In addition to meal delivery, N4L also provides Medical Nutrition Therapy and Nutrition Counseling to support overall wellness.
