Diabetes and Heart Health Services

Located in Takoma Park, MD, Diabetes and Heart Health Services is dedicated to empowering individuals with the knowledge and resources to prevent and manage diabetes and heart disease, conditions that disproportionately affect the African American community. With a focus on education, our program offers Chronic Disease Management Classes, support meetings, self-management counseling, and vital health screenings to promote well-being. We actively engage with the community by providing information and resources at events across Montgomery County, including health fairs and presentations, to raise awareness about the connection between these diseases. Funded by the Montgomery County DHHS and implemented by McFarland & Associates, Inc., our mission is to foster healthier lives through informed choices and proactive management.
