Case Management

In Severn, MD, the Case Management program at PDG is dedicated to connecting individuals with essential local resources and ensuring they receive all entitled benefits. Our comprehensive services include assistance with Social Security, Medicaid, Medicare, food stamps, and various forms of housing and energy aid. We also facilitate access to medical and dental care, psychiatric services, substance abuse treatment, medication management, vocational training, and transportation. By collaborating closely with each consumer, our compassionate Case Managers create customized support networks and advocate for the care and treatment needed to foster stability and empowerment within the community.
