Based in Salisbury, MD, Nutrition for Longevity (N4L) is dedicated to enhancing the health and well-being of individuals through our Home Delivered Meals program, including Medically Tailored Meals designed for clients eligible under Medicare Advantage and Medicaid Long-Term Services and Supports. Our meals, carefully crafted by registered dietitians, cater to specific medical needs, addressing conditions such as Chronic Kidney Disease and Congestive Heart Failure, while being free from dairy, peanuts, gluten, and pork. N4L is committed to making nutritious meals accessible; we partner with most Medicaid and Older Americans Act programs and also offer a convenient self-pay option for those not covered by insurance at just $7.75 per meal. Whether you have a Case Manager to assist with eligibility or are exploring options independently, our mission is to ensure that everyone has access to essential nutrition at home. Additionally, we provide Medical Nutrition Therapy and Nutrition Counseling to support our clients' overall health journey.
