Diabetes and Heart Health Services

Based in Rockville, MD, Diabetes and Heart Health Services is dedicated to empowering individuals with the knowledge and resources to prevent and manage diabetes and heart disease—two conditions that significantly impact the well-being of our community, particularly among African Americans, where 33% also struggle with heart issues. Our comprehensive program offers Chronic Disease Management Classes, Self-management Counseling, Health Screenings, and regular dMeetings to support individuals in their health journey. We also actively engage with the Montgomery County community by sharing valuable information at health fairs, walks, and presentations. Funded by Montgomery County DHHS and implemented by McFarland & Associates, Inc., our mission is to foster lasting health improvements and promote awareness about these critical health challenges.
