Diabetes and Heart Health Services in Potomac, MD, is dedicated to empowering individuals to take control of their health through comprehensive education and support aimed at preventing and managing diabetes and heart disease. Recognizing the critical connection between these two prevalent conditions—especially among African Americans—our program offers a variety of essential services, including Chronic Disease Management Classes, self-management counseling, and health screenings. We actively engage with the community by participating in health events throughout Montgomery County, providing valuable information and resources to promote wellness. With the support of Montgomery County DHHS, our mission is to foster healthier lives and enhance community awareness of diabetes and heart health.
