Diabetes and Heart Health Services

Located in Olney, MD, Diabetes and Heart Health Services is dedicated to providing essential education and resources aimed at preventing and managing diabetes and heart disease. Understanding the critical link between these two conditions, particularly within the African American community where approximately one-third of individuals with diabetes also face heart disease, we offer a suite of support programs including Chronic Disease Management Classes, support meetings, self-management counseling, and health screenings. Our organization actively engages with the community through various events across Montgomery County such as health fairs, walks, and educational presentations, ensuring vital information reaches those who need it most. Funded by Montgomery County DHHS and implemented by McFarland & Associates, Inc., our mission is to empower individuals with the knowledge and tools necessary for healthier living.
