Diabetes and Heart Health Services in Montgomery Village, MD is dedicated to empowering individuals through education and resources aimed at preventing and managing diabetes and heart disease—the two conditions that significantly impact our community, especially among African Americans, with 33% facing both. Our comprehensive programs include Chronic Disease Management Classes, self-management counseling, and health screenings designed to support personal health journeys. We actively engage with our community by participating in local events such as walks and health fairs to raise awareness about the importance of managing these interrelated conditions. Funded by Montgomery County DHHS and implemented by McFarland & Associates, Inc., we are committed to providing practical, expert guidance to enhance the health and well-being of those we serve.
