Cancer Transitions is a supportive and evidence-based program located in Joppa, MD, designed specifically for cancer survivors who have completed their treatment within the last 24 months, although individuals beyond this timeframe are also welcomed. Our mission is to empower survivors by offering vital resources that promote holistic health and well-being. Participants receive customized nutritional support, tailored exercise plans, and essential education on emotional and medical management strategies, all aimed at enhancing the quality of life and facilitating a smooth transition into survivorship.
