Cancer Transitions

Located in Havre De Grace, MD, Cancer Transitions is a dedicated program designed to empower cancer survivors who have completed treatment, particularly those within the past 24 months. Our evidence-based approach focuses on enhancing the quality of life for survivors through comprehensive support, including nutritional guidance, personalized exercise plans, and vital education on emotional health and medical management. We believe healing continues beyond treatment, which is why our resources are also available to those who are further along in their journey. Our mission is to equip each survivor with the tools and support needed to thrive in their survivorship.
