Diabetes and Heart Health Services

Diabetes and Heart Health Services in Gaithersburg, MD, is dedicated to empowering individuals with the knowledge and resources needed to prevent and manage diabetes and heart disease. Recognizing the critical link between these two conditions, especially in the African American community where 33% of those with diabetes also experience heart disease, our program offers vital support through chronic disease management classes, meetings, self-management counseling, and health screenings. We actively engage with the community by sharing essential health information at various events throughout Montgomery County, including health fairs and public presentations. Funded by the Montgomery County DHHS and implemented by McFarland & Associates, Inc., our mission is to foster healthier lives through education and accessible healthcare resources.
