Diabetes and Heart Health Services

Located in Derwood, MD, Diabetes and Heart Health Services is dedicated to empowering individuals with the knowledge and support needed to prevent and manage the interconnected issues of diabetes and heart disease. Recognizing the significant impact these chronic conditions have on our community, particularly among African Americans, we offer essential programs including Chronic Disease Management Classes, self-management counseling, and health screenings. We actively engage with the community by providing valuable information at various events across Montgomery County, such as health fairs and educational presentations. Funded by the Montgomery County DHHS and implemented by McFarland & Associates, Inc., our mission is to foster healthier lives through education and proactive health management.
