Diabetes and Heart Health Services

Diabetes and Heart Health Services, located in Damascus, MD, is dedicated to empowering individuals with the knowledge and support needed to prevent and manage diabetes and heart disease. Recognizing the critical connection between these two conditions, especially within the African American community where 33% face both challenges, our program offers a variety of essential services including Chronic Disease Management Classes, self-management counseling, and health screenings. We actively engage the Montgomery County community by providing valuable information at local events such as health fairs and walks, ensuring that everyone has access to the resources necessary for healthier living. Funded by Montgomery County DHHS and implemented by McFarland & Associates, Inc., we are committed to fostering a healthier future for all.
