Case Management

Based in Crofton, MD, the PDG Case Management program is dedicated to connecting individuals with essential local resources and ensuring they receive the benefits they deserve. Our comprehensive approach includes assistance with Social Security, Medicaid and Medicare, food stamps, housing support, energy assistance, and access to medical and dental care, as well as psychiatric care and substance abuse treatment. We recognize that each person's journey is unique, and our compassionate case managers partner with individuals to create personalized support networks that promote stability and well-being within their communities.
